Please print or download the following forms. All forms, with the exception of the medical form, should accompany your child's registration form and $75.00 deposit. The medical form is required one week prior to your child's camp arrival
Please utilize our medical form, which contains additional questions beyond a typical physical, as required by the New York State Department of Health for Children's Summer Camps. An immunization record can be attached to the medical form.
Ropes Course Authorization Form
By completing this form, you are authorizing your child to participate in our ropes course, are stating that the child has appropriate medical coverage and releasing Catholic Charities, Camp Nazareth and the Diocese of Syracuse from any and all liability relating to the ropes course.
Please review this form with your child. Our code of conduct conveys general expectations we have of camper attitude and behavior. Please note that both the camper and parent/guardian sign this form.
